The OSHA 300, 300A and 301 Forms are intended to recording injury and illness in a log, an annual summary, and incident reports. The files are in PDF format so it is best to save the document to your computer instead of using a plug-in viewer. Please note that the forms are designed to print on 8.5 x 14″ paper. To print on different sizes of paper, update your printer settings to fit the document onto 8.5 x 11″ or A4 paper.
