Application & Forms


An application for construction or modification of a public water supply system must be submitted to DWB whenever a new water system is constructed or an existing water system is modified. Please refer to our Construction webpage for additional information.

By October 1 of each year, all community water systems must submit a CONSUMER CONFIDENCE REPORT CERTIFICATION FORM to DWB to verify the distribution of the consumer confidence report.

All public water systems must complete an OPERATION AND MAINTENANCE PLAN, which should be reviewed and updated by the system annually, or whenever the system makes changes to source(s), treatment or distribution that affect system operation.

All public water systems must complete an EMERGENCY RESPONSE PLAN, which should be reviewed and updated by the system annually, or whenever the system makes changes to sources(s), treatment or distribution that affect system operation.

In order to become a water hauler, an APPLICATION FOR POTABLE WATER HAULING OPERATION must be submitted to DWB.

All surface water treatment systems are required to submit a MONTHLY OPERATING REPORT (MOR).   This link will take you to the MOR web page where you can download the spreadsheet.

Groundwater Rule Forms

Water systems that forgo triggered source water monitoring required by the Groundwater Rule due to distribution system issues must submit an EXEMPTION REQUEST FORM and seek approval from DWB within 24 hours of a total coliform positive sample.

Groundwater systems that provide 4-log treatment of viruses (either as a corrective action or to avoid triggered source water monitoring) must be certified by the DWB.  Systems desiring to be certified as providing 4-log treatment of viruses must complete the APPLICATION FOR GROUNDWATER RULE 4-LOG CERTIFICATION and provide all information requested by the DWB.


Disinfection by-products Stage 2 Forms

The Stage 2 DBPR establishes operational evaluation requirements that are initiated by the TTHM and HAA5 levels found during Stage 2 DBPR compliance monitoring. Compliance with Stage 2 DBPR MCLs is based on the average of four individual quarterly DBP measurements collected at a given location (i.e., LRAA). However, a system that is in compliance with the Stage 2 DBPR MCLs, based on the LRAA, at a location may still have individual (i.e., not averaged) DPB measurements at that location that exceed the Stage 2 DBPR MCLs. EPA and the Stage 2 Microbial/Disinfection Byproducts (M/DBP) Advisory Committee were concerned about these higher levels of DBPs. The Stage 2 DBBR operational evaluation requirements were established to address these concerns.

The Stage DBPR requires systems to conduct operational evaluations, initiated by the operational evaluation levels (OEL) found in Stage 2 DBPR compliance monitoring, and to submit an operational evaluation report to the State.  The OELs initiate a comprehensive review of system operations and act as an early warning for a possible Stage 2 DBPR violation in the following quarter. This early warning allows systems to act to prevent the violation.
